​Ear Wax Removal
Ear syringing by general practice has never been funded in our area and based on national advice from the BMA the decision was made that wef 1/11/2021 to stop carrying out ear syringing at Scorton Medical Centre
Management

Ear syringing is no longer considered to be the first line treatment for the clearing of ear wax; current guidelines are that ear drops should be used to soften the wax which will then enable the natural movement of the wax from the ear.
Patients who do suffer from a build-up of wax, should regularly apply drops to prevent such a build-up and should avoid putting objects in the ears to try to manually remove wax. This can impact the wax by pushing it further down the ear canal.
It is recommended that you speak to a pharmacist about earwax build-up in the first instance. They can give provide advice and suggest treatments. They might recommend chemical drops to dissolve the earwax. Otex Express sodium bicarbonate is highly recommended by clinical staff and patients have found it to be effective but there are other options available which a pharmacist can discuss with you.
Removing wax professionally
Where self-management does not resolve the wax build up microsuction is the method preferred by medical staff and patients alike for guaranteed effective removal of wax. In most case the wax can be removed safely within minutes by a trained professional. Below are a number of private providers in the area:

Patients who require wax removal for specific ear conditions will continue to have this performed in a hospital setting.
Any previous hole in your eardrum (a perforated eardrum), sudden hearing loss, earache and/or fluid within the ear a telephone consultation with a GP should be requested.

Weight Management Service

Choose to lose
The ‘Choose to Lose’ scheme run by Richmondshire District Council is offering online classes and groups so members who join the programme without the worry of travelling to classes, missing class times, and finding a group that suits lifestyle and commitments.

The free of charge programme is for adults with a BMI above 25 who are in need of support to implement a healthier lifestyle. Funded by North Yorkshire County Council Public Health it is made up of a 12 week structured weight management programme plus a 12 week maintenance programme for those who achieve 5% weight loss in their first 12 weeks.

The virtual service will provide both aspects of the programme as well as access to a Weight Loss Adviser, online live classes, chat groups and Facebook open and closed groups.

This service is free to anyone who meets the following criteria;
  • aged 18 years or over
  • BMI (Body Mass Index) equal to or greater than 30 or 25 with other health conditions

Extended LARC use:
Current National Guidelines support extended use of contraception's as the risk of pregnancy is though to be very low but evidence for this is limited.  Current guidelines advise that the implant can be used for 4 years: Mirena /  Levosert IUS 6 yeas, T-safe 380 copper coil 12 years and Nova T copper coil 6 years.  If patients are concerned about extended use they can use condoms or a progesterone only pill in addition to their LARC method.

Fit For Work

This is a new organisation, funded by the Department for Work and Pensions. They offer a free service to help people who are in paid employment (or self employed) who have been or are going to be off work because of an illness or operation for at least 4 weeks. Your GP or your Employer can refer you. There is also some helpful advice for employees on the website. They can issue sick/fit notes in the same way as GPs can and they can liaise with employers to help people return to work.

Improving Access to Psychological Therapies

IAPT is a national NHS programme designed to increase the availability of talking therapy treatments recommended by the National Institute for Health and Clinical Excellence (NICE).  Significant events like bereavement, unemployment, relationship breakdown, traumatic events or even stress at work can lead to difficulties requiring help and support. Problems with low mood and anxiety can develop and make it difficult for us to cope with life’s daily demands.

To make sure that the IAPT service is right for you, you will need to complete a self-referral form. You can self-refer here or via your GP. Assessments are undertaken over the phone, although alternative arrangements can be made if required. At the end of the assessment, we will discuss what help is available within the service.

Where IAPT is not the most suitable service we can signpost you to alternatives where available.

​For those living in County Durham and Darlington, Teesside, North Yorkshire and York. The new, all age, single point of access telephone number is available 24 hours a day, seven days a week; making it easier to get crisis support when and where you need it.
The phone line, which is open 24 hours a day, seven days a week, offers callers, including those with learning disabilities and/or autism, a series of options which will divert them to their local service.

Community Eye Care Service

This service is now available to offer to all patients over the age of 2 years. 
The Community Eye Care Service (CECS) provides a Minor Eye Conditions Service (MECS) and a Glaucoma Repeat Readings service (GRR) patients can now call 0300 303 4922, Monday to Saturday 9am-5pm. Patients will be triaged by the Primary Eyecare Team and assessed accordingly either by telemedicine or by an arranged face to face consultation.
